Types of Care

Nursing care is offered to residents who need 24-hour around the clock care. This will be set out in a residents’ individual care plan and includes the monitoring and assessment of potential clinical problems such as: pressure area management; nutritional balance; fluid balance; administration and management of medication and pain management.

Residential care is long-term care provided to adults who are staying in a residential setting rather than in their own home. Our homes provide accommodation, meals, daily activities and individual care such as personal hygiene and nutrition. Each of our residential care homes is unique, designed to make the most of the home environment and its surroundings and with a focus on making sure that residents lead a happy, fulfilling and healthy life whilst in our care, in a home they feel safe and secure in. We want our homes to be their homes.

We Care Group provides dementia care in both a residential and nursing setting. Each of our homes that support people living with dementia is uniquely designed to make residents feel comfortable in their homes. Corridors are filled with photos and murals to evoke fond memories from the past as well as memorabilia to encourage stimulation. Colours are used throughout the home to create positive associations and trigger the right response to a situation and the food and menus are designed to cater specifically for residents with dementia. Several of our homes even have a dementia garden creating a sensory experience in a beautiful setting.
Specialist teams are on hand day and night to provide emotional and practical support to our residents, making sure they feel completely at home in their environment. Each resident’s personalised care plan is designed to support them in living with dementia in a positive way and in addition to any medical requirements, covers everything from mealtimes, hobbies and habits to activities that will stimulate their senses and engage them in a way that is fulfilling whilst at all times respecting a resident’s privacy and independence.

Palliative care is provided predominantly in our nursing homes and is focused on managing pain and making your loved one as comfortable as possible, ensuring they receive the care they want, where and how they want it, with dignity and respect in their final moments.

Our caring and empathetic teams provide highly sensitive and compassionate end-of-life care within our nursing homes, working with your loved one and their family, as well as their GP and other health professionals, to plan their care.

Bariatric care is provided in both a residential and nursing setting depending on a resident’s needs. Our teams are trained to support bariatric residents with their mobility, provide the right type of care to meet individual needs (including for example, keeping wounds clean), as well as supporting bariatric residents with their daily tasks. Our chef teams make sure dietary requirements and plans are followed whilst also being considerate to any preferences our bariatric residents may have.

We said: Staff meetings had been held and regular daily handovers supported staff to keep up to date. We found these meetings could be more effective.

You did:

Daily flash meetings are held with the team to ensure there is effective targeted communication between departments. Team meetings and Clinical Governance meetings are held with a standard agenda to ensure that all necessary items are shared, discussed and that any lessons are learned and used to inform our care.

We said: There were a high number of agency staff working in the home, including nurses. The provider was recruiting to permanent positions in the home and this was ongoing.

You did:

All permanent positions have been recruited into and agency workers are only used to cover annual leave and sickness if any of our own team are able to cover. Where possible we use the same agency team member so that the home, our policies and procedures and ways of working, promoting safe and effective care for our residents.

We said: People did not receive consistent high-quality person-centred care because managers had not ensured there was enough oversight of the quality of care and care records.

You did:

As part of our Governance Framework, a cycle of monthly audits are undertaken which monitor the quality of care our residents receive and the associated documentation. A monthly review of key care indicators monitor resident outcomes and are used to inform practice and promote resident well-being.

We said: People and their relatives had mixed views about how well the home was managed. The home's regular staff were praised as being knowledgeable and caring. People's relatives did not feel they had been asked for their feedback recently however, this was an area the provider had already identified as needing to be addressed.

You did:

Loved ones are included in the review of resident’s care plans together with the resident. There are regular residents and relatives meeting with the Home Manager, and also opportunities for relatives to meet with the Home Manager on an individual basis.

We said: We found people were not deprived of their liberty without appropriate legal authorisation either applied for or in place. Our records included some mental capacity assessments in relation to some particular decisions people needed to make, this included receiving personal care, support with medicines and living in the home. All residents that lack capacity need to have a decision specific CA/BI meeting to determine any restrictions in the home that could deprive them of their liberty.

You did:

All decision specific capacity assessments and best interests decisions are in place for those residents who need them.

We said: Information from other professionals had been included in some care records and plans of care. However, we found it difficult to access copies of letters from other professionals and were not assured their advice had been accurately included in care plans.

You did:

All letters from professionals are held in the residents electronic file on our care planning platform. Care plans have been reviewed and written to reflect the advice of the other professionals.

We said: People's needs had been assessed using an electronic system. Staff accessed care records on handsets. Some assessments and care plans did not contain enough information to guide staff.

You did:

Keyworkers have been assigned to direct the care of the residents, they fully understand the needs of the residents and regularly discuss any care plan updates required with the residents and their loved ones.

We said: Information to guide staff when to give medicines prescribed 'when required' including pain relief, inhalers and sprays for angina, were not in place. People were at risk of not receiving these medicines properly.

You did:

All “as required” medication has an associated protocol so that the team know how to recognise the signs and symptoms that each resident may display when requiring the medication.

We said: Diabetes care was inconsistent. Care plans to support people with diabetes lacked sufficient detail and were not person centred. One person's insulin regime was incorrect.

You did:

All residents with insulin dependent diabetes are reviewed by the diabetic nurse and any changes with their dose or regime are emailed from the diabetic team and reviewed with the GP to ensure the correct insulin and dose is prescribed. All care plans have been written to reflect NICE guidelines and the individual needs of the resident.

We said: Some people had not received their medicines as prescribed. Some medicines were not always in stock. Medicine records did not provide accurate information about stocks because balances had not been transferred to new medicine administration records, (MAR) each month.

You did:

The home now has an electronic medication system which monitors the stock balance of all medications and prompts the team to order more medication if stocks are running low.

Our auntie and friend was a resident for 10 years at Swansea Terrace. Thankfully throughout her time at the above nursing home, all staff worked hard to preserve her independence and respect her dignity. As her physical health deteriorated, appropriate care was always available. Despite changes in ownership during the 10 year period, we were always confident that this did not impact our relatives, due to the very caring staff. We visited frequently and can confidently say that every aspect of her care was met. She died peacefully in February, having received excellent end of life care.

When my wife was admitted to Swansea Terrace in mid-September 2017 after breaking her hip and also suffering from dementia, we were told by surgeons the chances of her walking again were remote. She was 5 1/2 stone then, now she's 8 stone and can walk unaided. No need for a sling to get Mum up now, thanks to

Swansea Terrace Care Home is part of a group that runs successful Nursing and Residential Care Homes. The Home is registered and inspected by Care Quality Commission (CQC) to provide outstanding care as follows:

  • Caring for adults over 65 years
  • Caring for adults under 65 years
  • People living with dementia
  • Physical Disabilities
  • People who require Nursing or Personal care
  • Treatment of disease, disorder or injury.

The maximum number of Residents allowed at any one time is 44.

Swansea Terrace Care Home aims to continually provide its residents with a secure, relaxed, and homely environment in which their care, well-being and comfort is of prime importance.

Carers will always strive to preserve and maintain the dignity, individuality and privacy of all residents and in so doing will be sensitive to the residents’ ever-changing needs. Such needs may be medical / therapeutic (for physical and mental welfare), cultural, psychological, spiritual, emotional and social, and residents are encouraged to participate in the development of their individualised Care Plans in which the involvement of family and friends may be appropriate and is greatly valued.

All Care staff within the home will be appropriately qualified to deliver the highest standards of care. A continuous staff training programme is implemented to ensure that these high standards are maintained in line with the latest initiatives and developments in Care Practices as may be laid down in appropriate legislation, regulations and Care Standards.

All of our bedrooms are bright and airy, and residents are encouraged to personalise their bedroom as much as possible so that it feels like a home away from home.

We provide a nurse call bell and TV point in all bedrooms.  Residents can also purchase individual telephone services/Sky TV packages for their bedrooms if they wish to do so.

The home is accessible for wheelchair users and handrails are available in all communal areas, and we have recently installed CCTV in communal areas and access points for the safety of our residents.

We have a range of specialist equipment to meet the needs of our residents including specialist mattresses and beds, full-body hoisting equipment, standing hoist equipment, and a “Sam Hall” turner to help residents when transferring.

We have a range of adapted bathing and shower facilities.

 

Our chef teams use great ingredients to provide freshly prepared, nutritionally balanced meals based on regularly changing, seasonal menus. We make mealtimes an occasion for our residents, enabling them to enjoy their food in welcoming surroundings and catered for by our care teams. Residents always have multiple options to choose from at all meal times and can opt for something different if they change their minds when the food arrives.

We make sure our residents start the day right with a filling breakfast including cereals, fruit, toast and porridge, plus a daily changing choice of hot options. Our Residents are welcome to have breakfast any time they desire, and we are happy to accommodate their morning schedule. Between Breakfast and Lunch, we offer mid-morning cake and coffee.

Here lunch is the main meal of the day – enabling residents to dine together in a restaurant-style environment and providing a choice of hot meals and the option of soup or a sandwich for those that don’t want something quite as filling. Lunch is finished with delicious desserts, all prepared on-site by our chef teams.

And then, between lunch and supper, it’s mid-afternoon tea time with mouth-watering freshly baked cakes that our residents and team members enjoy together, making afternoon tea an activity in itself. After supper, during the night, lighter meal options are provided in the evening, and fresh fruit and snacks are available throughout the day to keep resident’s nourished, energised and satisfied.

Management Team

Rachael Harrison is the Registered Manager of Swansea Terrace. Rachael oversees the day-to-day running of the Home. The Deputy Manager supports this role and takes charge in the absence of the Manager. The Registered Nurse,  Senior Carers and Carers tend to care for the Residents.

Care Team

Swansea Terrace has approximately 48 members of staff. The majority of our care staff hold an NVQ level 2 or above in Health and Social Care.

Support Team

The Admin and Head Office team are responsible for dealing with fees and clients personal allowance, suppliers’ invoices and payroll.

Domestic and Maintenance staff ensure the home is a safe and clean environment at all times, following the regulations set by COSHH, HSE and IPC.

Our cooks work on alternate days preparing and providing meals to our residents 3 times a day to meet each individual’s dietary need.

Swansea invests in a wide range of training for all our staff members, not only do we complete the mandatory training, but we also complete additional training courses such as Catheter Care, Challenging Behaviour, Malnutrition Universal Screening Tool, Dysphasia, Common Health Problems, End of Life Care and much more. All our staff re-train every year in dementia to keep our knowledge up to date to ensure we provide the best care.

Therapeutic activities are available for all our residents including those living with dementia; these include the normal recreational events. Activities are planned and actioned through our Activities Co-ordinator; a 4 weekly plan is available.

Every resident is unique, and their activities should be too. Oomph! helps our team create personalised experiences based on each person’s interests, hobbies and abilities. From music, arts and crafts and baking to gentle exercise, gardening and reminiscence sessions, our varied programme encourages residents to stay active, engaged and connected.

Taking part in meaningful activities supports both physical and emotional wellbeing. It helps maintain mobility and confidence, encourages friendships, reduces feelings of loneliness and provides valuable mental stimulation. Most importantly, it enables residents to continue enjoying the things that bring them joy.

For families, this means the reassurance of knowing their loved one is living each day with purpose. By spending less time planning and more time with residents, our wellbeing team can focus on creating moments that matter, helping every person feel valued, included and truly at home.

What happens if I can no longer afford private care?

If your circumstances change and you become eligible for local authority funding, we will work closely with you, your family, and the funding authority to ensure your continued care. While some public funding may not cover the full cost of your current room and services, we will explore all possible solutions; such as third-party top-ups or room changes where appropriate. Most importantly, we do not serve notice to residents purely because of a transition from private payment to local authority funding. We are committed to providing a long-term home and supporting you every step of the way.
